Your Next Visit

To make the most of your visit, please bring the following to your next appointment:

  • A current copy of your insurance card/cards
  • A list of your current medications and refill status
  • The name of the pharmacy where you wish to have your prescriptions filled
  • A list of all providers that you see and the diagnosis that you are treated for

If this is your first visit, please bring...

  • Completed registration forms
  • Important: Please contact your current medical provider and have your medical records released to our office.  It is important that we have your records prior to your visit if at all possible.  

Appointment information

We encourage you to call as far in advance as possible to schedule any routine appointment needs.   For more urgent, or "sick" visit needs, we offer same day appointments.  We have “walk-in” care available for our established patients as well from 8-11am Monday through Friday on a first come –first serve basis.  Walk in hours are staffed by one of our Nurse Practitioners.   As your Medical Home, appointments are made with your Primary Care Provider or Care Team to the best of our ability.  The office opens daily at 7:00AM - some early morning and evening hours are available for your convenience.  We also provide Saturday morning hours for those who cannot make an appointment during the week.  

When you need to cancel your appointment

If you are unable to keep your appointment, please call the office as soon as possible.  A 24-hour notice is appreciated - this courtesy allows us to be of service to other patients.  Please know, if you are late for your appointment, you may be asked to reschedule for another day.  Also, it is important to note that repeat no-show patients may be asked to find another primary care physician. 

Payment information

Payment is expected at the time of service.  This includes your co-pay or any coinsurance that you may have.  We accept cash, check, debit and all major credit cards.

Referrals

If your policy is referral based, it is your responsibility to know which services require a referral or pre-certification and whether or not the specialist you wish to see is covered by your plan.  Always check with your insurance company first if you have questions concerning the referral process, especially if you have made appointments outside of this office on your own.  We will make every effort to process your referrals promptly but cannot guarantee a last minute referral.  We will work with you as your Care Team to be certain that your services will follow the protocol of your insurance carrier. 

Prescription renewal

Please give 24 hours of notice for prescription refills.  To make sure you do not run out of medication, please do not wait until you are out of medication to call us.   Emergency requests for prescription refills will be limited to a 7-day supply.  Controlled medications cannot be renewed after hours. 

Web Portal

Once you are established as a patient, you will be offered access to our secure Web Portal where you can  view your results, ask medical questions, request medication refills, or request referrals.  Please feel free to ask our staff about this option should you have any questions.
