Community and Population Health

The Community Health team supports health improvement initiatives through a planned approach of substance abuse, tobacco use, secondhand smoke and obesity prevention. Choose To Be Healthy is a coalition that works on Healthy Maine Partnership and Drug Free Communities grant strategies. Partners include municipalities, schools, law enforcement, retailers and food security organizations. While most of the work is done in the York Hospital service area, support is also provided to the York Public Health District (York County). This collaboration allows for identifying and prioritizing health needs.

York County Physical Activity Resource Guide

The Community Health Team worked along with two other Healthy Maine Partnerships within York County to present a Physical Activity Resource guide that includes all 29 communities.

It is the hope of the partnerships that this resource guide will encourage physical activity among people of all ages in the area.
