York Hospital Home Care is not limited to York Hospital patients:
The team accepts referrals for evaluations and skilled home care regardless of healthcare system or hospital, for all patients living in the Yorks, Kittery, Eliot, Wells, Moody, Ogunquit, the Berwicks and Kennebunk.
York Hospital Home Care provides services 7 days a week. An intake nurse is available to help with patient care referrals Monday through Friday, 8am-5pm and on-call nurses are also available 24 hours a day. Once a referral is made, a York Hospital clinician will contact you within 24 hours of referral or discharge from a facility and will arrange a visit within 24-48 hours of discharge. Please note, physician orders are required for any treatment that is provided.
