For the fifth straight year, York Hospital Pediatric, Primary Care and Internal Medicine practices have received “Patient-Centered Medical Home Recognition” by the National Committee for Quality Assurance (NCQA). The NCQA Patient-Centered Medical Home program assesses whether clinician practices are functioning as medical homes, emphasizing the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement. It reflects the input of the American College of Physicians, American Academy of Family Physicians, American Academy of Pediatrics, American Osteopathic Association, and others.

The York Hospital practices recognized are Great Works, Kittery, Sanford and Wells Family Practices, Pediatric Associates of York Hospital, Internal Medicine of York and Webhannet Internal Medicine.

“The primary care, pediatric and internal medicine teams at York Hospital have worked diligently to consistently uphold the best practice standards of the NCQA patient-centered medical home for five years running. The NCQA is a private, nonprofit organization whose mission is to improve the quality of health care, accrediting and certifying a wide range of health care organizations. It also manages the evolution of HEDIS®, a performance measurement tool used by more than 90 percent of the nation’s health plans.
