­­­We are pleased to report that York Hospital’s Family Medicine, Internal Medicine and Pediatric practices recently received National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home recertification, valid through April 2022. Dr. Jennifer Cutts, Chief Medical Officer and Director of Quality, states that this recognition affirms the hospital’s commitment to best practices and the highest standards of quality, noting: “Our caregivers understand the importance of setting goals and working hard to achieve those goals. This recognition reflects the care and pride in their work that our caregivers bring to York Hospital every day.”

Richard Perrotti, Director of Practice Operations, adds: “This certification lets our patients know that York Hospital is committed to the highest quality of care and continuous improvement to our care delivery. Special thanks to Tracy Colburn, Kelly Poole and Fia Yarian for their leadership and tireless work these past few months. Their efforts assured that we were well poised to receive this recertification.”

Practices recognized include Family Medicine in Wells, Family Practice in Sanford, Great Works Family Practice, Kittery Family Practice, Pediatric Associates, Webhannet Internal Medicine, York Family Practice, and York Internal Medicine.

NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement, to build better relationships between patients and their clinical care teams. NCQA, a private, non-profit organization dedicated to improving health care quality, accredits and certifies a wide range of health care organizations and manages the evolution of HEDIS®, a performance measurement tool used by more than 90 percent of the nation’s health plans.
