At York Hospital, our care management team is comprised of Social Workers and RN’s who work together to assist patients and families with social, emotional and financial needs. Our team collaborates with patients, families and all members of the patient care team to provide an optimal experience while an inpatient or in the emergency room. The nurses and Social Workers provide information, referral counseling, crisis intervention, care management and discharge planning services.
Based on physician orders, our team arranges for such things as home care, hospice, medical equipment, insurance companies and other assistance. We are here for you! Please feel free to contact us while you are here or if you have any questions or concerns.
Advance care planning may also be an important part of a patient’s care management plan. An advance care plan provides instructions about the healthcare you want, as an adult 18 years of age or older, if you become too hurt or ill to speak for yourself. York Hospital offers a downloadable advance directive form as well as a downloadable reference guide to assist patients in navigating the process of documenting final wishes.

CARE ACCESS NURSE
York Hospital’s “Care Access Nurse” guides patients and families through the health care system by establishing contact with clinicians, patients, and families to improve access to care. The Care Access Nurse offers help with access issues, developing relationships with the appropriate medical staff, and maintaining open communication between all individuals involved in the care of the patient.
