Giving you more control over your care
At York Hospital, we offer two secure online portals for our patients. The first is our Practice Portal, which allows primary and specialty care patients to access medical records, get lab and imaging results, view medications, manage appointments, message their providers, and more. The second is our Hospital Portal (also known as FollowMyHealth), which gives you access to medical records for any services received at York Hospital, even if you’re not a practice patient.
New Hospital Portal Coming Monday, April 13!
We're switching to a new hospital patient portal, FollowMyHealth (FMH), on April 13, 2026. You will receive an email with instructions on how to activate and access your new FMH account. Your existing medical records will be included in your new FMH record.
Practice Portal (Healow)Hospital Portal (FollowMyHealth)
On April 13, the current hospital portal, Syntellis, will move to view-only mode. No new medical information will appear there after this date. Full access to Syntellis will end on April 30, when the platform is retired. All new clinical information will be available exclusively through FollowMyHealth.
This change affects only the hospital portal. The practice portal (Healow) will continue to be used for practice patients and is not part of this transition.

In keeping with federal rules, York Hospital provides patients with immediate access to certain health care information in their electronic medical record through our patient portals. This means you may see your test results at the same time as, or even before, your provider.
Please note that you don’t have to review your results on the portal if you don’t want to. You may also receive your results from your provider. If you have questions about your results, please talk with your provider.

MyLinks
MyLinks is a free app that lets patients consolidate their medical records from facilities outside York Hospital in one convenient and secure location.
Want access to telemedicine?
York Hospital provider practices offer secure online telemedicine visits so that you can be treated, consulted, and advised by your provider from the comfort of your home. To schedule a telemedicine appointment, simply set up your Practice Portal and call your provider practice.
