The Center for Community Health Promotion is a component of York Hospital that supports the following health improvement and health access initiatives in the Southern York County area. The Community Health Promotion team implements a planned approach to providing access to health care and improving the health of those who live in our communities. Working together with diverse community stakeholders we assist in identifying health needs and priorities and work to address those needs and priorities.
York Community Health Promotion has brought over $2 million of foundation, state, and federal funding to address identified community health priorities.

Community Health Connection is the central health access program of York Community Health Promotion. Recognizing that access to health care is fundamental to the well-being of all community members, we support a network of free health services for those without insurance and who have low family incomes living in Kittery, Eliot, York, Wells, Ogunquit and South Berwick. The network of health care services includes hospital in-patient and out-patient services, primary care and specialty physicians, some alternative care providers, and mental health professionals. In response to community need, YCHP is also working with a coalition of organizations county-wide to address dental health needs. This service also includes a prescription assistance program and medication management program.

The Choose To Be Healthy Partnership is part of the State of Maine's foremost public health initiative, the Healthy Maine Partnerships, funded by tobacco settlement funds through the Bureau of Health. The project takes the lead in reducing the incidence of chronic diseases in our service area. Schools, health care providers, and worksites engage in collaborative actions leading to policy and environmental changes that reduce the burden of these diseases. This program includes a Leadership and Resiliency Program. Current local priorities include establishing coordinated school health programs in 3 school systems, increasing the availability of help for quitting smoking, and preventing childhood obesity.

Our youth advocacy program trains youth to be advocates for healthy lifestyles. These youth leaders work to promote physical activity, tobacco and alcohol resistance, and proper nutrition, to youth in our community. They have helped to influence the passage of strict municipal tobacco policies and are now training similar teen groups across the state in how to implement successful education and awareness events as the first step toward policy change.
